Entry Forms

 2008 Offshore Experience Form 
(Electronic Word version - can be completed in Word and e-mail to Participation Committee.)  Can be requested of any boat that files an  Application for Entry.

(PDF Form - can be completed by hand and faxed to Participation Committee)  Can be requested of any boat that files an Application for Entry.
 Captain's Indemnification & Waiver Form
Must be completed by all Captains prior to 1700 May 15.

Trophy Eligibility Forms

 Family Prize Entry Form 
Eligible boats must have four family members aboard.  See form for additional details and deadline.
 Pratt Trophy 3-Boat Team Entry Form
Self-selected three-boat teams in the St. David's Division.  See form for additional details and deadline. 

Post-Race Forms

 Accident, Injury & Illness Report
Must be completed by all boats after finishing in Bermuda.  Yachts not finishing are asked to mail or fax their report to the race office in Newport.
 Certificate of Compliance
Must be completed by all yachts after finishing in Bermuda.  Yacht will not be scored until Certificate of Compliance is completed, signed and delivered to Duty Desk.  See form for additional details. 
  Bermuda Tourism Survey Card
Must be completed by all yachts after finishing in Bermuda, and turned in to the duty desk at the RBYC.
